Cat Grooming Waiver
If you are bringing your cat to us to be groomed, this form is required in addition to the New Client form.
Double-coated dog shavedown/short haircut waiver
If you have requested that we shave down or give your double-coated dog a short haircut, you are required to complete the shavedown waiver each time the service is performed.
